Darren De Souza left us after four years helping our patients with supported pain management and other long term conditions.
Community's Mandy Gibbons-Phelan, said: " Darren remained determined and focused in supporting and coordinating supported self management. It originally was aimed at pain management but courses are generic and cater for people with all sorts of long term conditions and they are delivered in the community.
"Initially Darren coordinated the groups but then delivered the courses with staff and volunteers.
"He has significantly grown the group of volunteers and it will be a big wrench to leave them behind.
"Darren should be very proud that the supported self management groups he and other staff have delivered are life changing for the people who attend . He leaves a good platform to continue this very important support to people of Calderdale. We wish him every luck in his new post in pharmacy public health West Yorkshire."
